ABSTRACT

BACKGROUND@#Whether high cut-off (HCO) membranes are more effective than high-flux (HF) membranes in patients requiring renal replacement therapy (RRT) remains controversial. The aim of this systematic review was to investigate the efficacy of HCO membranes regarding the clearance of inflammation-related mediators, β2-microglobulin and urea; albumin loss; and all-cause mortality in patients requiring RRT.@*METHODS@#We searched all relevant studies on PubMed, Embase, Web of Science, the Cochrane Library, and China National Knowledge Infrastructure, with no language or publication year restrictions. Two reviewers independently selected studies and extracted data using a prespecified extraction instrument. Only randomized controlled trials (RCTs) were included. Summary estimates of standardized mean differences (SMDs) or weighted mean differences (WMDs) and risk ratios (RRs) were obtained by fixed-effects or random-effects models. Sensitivity analyses and subgroup analyses were performed to determine the source of heterogeneity.@*RESULTS@#Nineteen RCTs involving 710 participants were included in this systematic review. Compared with HF membranes, HCO membranes were more effective in reducing the plasma level of interleukin-6 (IL-6) (SMD -0.25, 95% confidence interval (CI) -0.48 to -0.01, P   =  0.04, I2  = 63.8%); however, no difference was observed in the clearance of tumor necrosis factor-α (TNF-α) (SMD 0.03, 95% CI -0.27 to 0.33, P  = 0.84, I2  = 4.3%), IL-10 (SMD 0.22, 95% CI -0.12 to 0.55, P  = 0.21, I2  = 0.0%), or urea (WMD -0.27, 95% CI -2.77 to 2.23, P  = 0.83, I2  = 19.6%). In addition, a more significant reduction ratio of β 2 -microglobulin (WMD 14.8, 95% CI 3.78 to 25.82, P  = 0.01, I2  = 88.3%) and a more obvious loss of albumin (WMD -0.25, 95% CI -0.35 to -0.16, P  < 0.01, I2  = 40.8%) could be observed with the treatment of HCO membranes. For all-cause mortality, there was no difference between the two groups (risk ratio [RR] 1.10, 95% CI 0.87 to 1.40, P  = 0.43, I2  = 0.0%).@*CONCLUSIONS@#Compared with HF membranes, HCO membranes might have additional benefits on the clearance of IL-6 and β 2-microglobulin but not on TNF-α, IL-10, and urea. Albumin loss is more serious with the treatment of HCO membranes. There was no difference in all-cause mortality between HCO and HF membranes. Further larger high-quality RCTs are needed to strengthen the effects of HCO membranes.

ABSTRACT

RESUMEN La insuficiencia renal aguda es definida como la pérdida de función del riñón ocasionada por diversas causas, entre ellas infección e ingesta de fármacos. Esta entidad tiene alta morbilidad y mortalidad en las unidades de cuidados críticos. El tratamiento de la misma va desde la propia protección renal hasta la sustitución artificial de las funciones del riñón lesionado. En la actualidad la terapia de reemplazo renal continua se ha utilizado como soporte renal, y ofrece mayor estabilidad clínica a los pacientes más inestables. En esta revisión se comentan conceptos, indicaciones y los más recientes estudios que validan el uso de esta terapéutica, así como el método de programación que se utilizó en un paciente con diagnóstico de una leptospirosis icterohemorrágica (síndrome de Weil), que estuvo en shock séptico con disfunción multiorgánica, donde se empleó esta terapia con resultados satisfactorios (AU).


ABSTRACT Acute kidney failure is defined as the loss of kidney function caused by various causes, including infection and drug intake. This entity has high morbidity and mortality in critical care units. Treatment ranges from renal protection to artificial replacement of the functions of the injured kidney. Currently, continuous renal replacement therapy has been used as renal support, and offers greater clinical stability to the most unstable patients. In this review, authors discuss concepts, indications and the most recent studies that validate the use of this therapeutic, as well as the programming method that was used in a patient with diagnosis of icteric-hemorrhagic leptospirosis (Weil syndrome), who was in septic shock with multiorgan dysfunction, where this therapy was used with satisfactory results (AU).

ABSTRACT

Abstract This patient was a 73-year-old man who initially came to our service with acute respiratory failure secondary to COVID-19. Soon after hospitalization, he was submitted to orotracheal intubation and placed in the prone position to improve hypoxia, due to severe acute respiratory syndrome (SARS). On the third day of hospitalization, he developed acute oliguric kidney injury and volume overload. The nephrology service was activated to obtain deep venous access for renal replacement therapy (RRT). The patient could not be placed in the supine position due to significant hypoxemia. A 50-cm Permcath (MAHURKARTM, Covidien, Massachusetts, USA) was inserted through the left popliteal vein. This case report describes a possible challenging scenario that the interventional nephrologist may encounter when dealing with patients with COVID-19 with respiratory impairment in the prone position.


Resumo O paciente era um homem de 73 anos de idade que inicialmente veio ao nosso serviço com insuficiência respiratória aguda secundária à COVID-19. Logo após a internação, ele foi submetido à intubação orotraqueal e pronado para melhorar a hipóxia devido à síndrome respiratória aguda grave (SARS - do inglês "severe acute respiratory syndrome"). No terceiro dia de internação, o mesmo desenvolveu lesão renal aguda oligúrica e sobrecarga de volume. O serviço de nefrologia foi acionado para realizar acesso venoso profundo para terapia renal substitutiva (TRS). O paciente não pôde ser colocado na posição de decúbito dorsal devido a uma hipoxemia significativa. Um Permcath de 50 cm (MAHURKARTM, Covidien, Massachusetts, EUA) foi inserido através da veia poplítea esquerda. Este relato de caso descreve um possível cenário desafiador com o qual o nefrologista intervencionista pode se deparar ao lidar com pacientes com COVID-19 com problemas respiratórios e colocados em pronação.

ABSTRACT

La sarcoidosis es una enfermedad granulomatosa no caseificante, multisistémica, de causa desconocida, que compromete el pulmón y los ganglios linfáticos mediastinales entre el 90-95 por ciento de los casos. También puede afectar otros órganos como las glándulas salivales, piel, ojos, hígado, bazo, corazón, huesos y sistema nervioso central. El compromiso torácico representa la principal causa de morbilidad con unas 10/100 000 personas al año y mortalidad. Nuestro estudio tuvo como objetivo presentar a un paciente de 45 años de edad atendido por tener manifestaciones clínicas, paraclínicas y radiográficas que permitieron establecer, sin confirmar mediante estudio histológico, el diagnóstico de una forma aguda de sarcoidosis. Su forma clínica de expresión coincidió con el síndrome de Löfgren con ensanchamiento mediastinal bilateral por linfoadenopatía hiliar, síndrome febril, eritema nudoso y artritis aguda en miembros inferiores, que desarrollan entre un 20-30 por ciento de los pacientes con sarcoidosis. Se le realizó una tomografía por emisión de positrones, cuyos resultados ofrecieron elementos diagnósticos propios de un proceso inflamatorio pulmonar alejado de las características propias de una neoplasia maligna. Después de una profunda evaluación del riesgo-beneficio, se impuso tratamiento inmunosupresor con esteroides a dosis elevadas y seguimiento estrecho. Observamos una evolución muy positiva e inmediata desde el punto de vista clínico y radiográfico. Luego de varias semanas de tratamiento se apreciaron los cambios con marcada mejoría hasta pasados 3 meses que constatamos prácticamente una casi desaparición de las imágenes radiográficas(AU)


Sarcoidosis is a multisystemic, non-caseating granulomatous disease of unknown cause that involves the lung and the mediastinal lymph nodes in between 90 percent and 95 percent of cases. It can also affect other organs, such as the salivary glands, skin, eyes, liver, spleen, heart, bones, and central nervous system. Thoracic involvement represents the main cause of morbidity and mortality in patients with this entity, which occurs in about 10 / 100,000 people a year. Our study aimed to present a 45-year-old patient treated for presenting clinical, paraclinical and radiological manifestations that allow the diagnosis of an acute form of sarcoidosis to be established without confirming by histological study. Its clinical form of expression showed the presence of Löfgren's syndrome with bilateral mediastinal widening due to hilar lymphadenopathy, febrile syndrome, erythema nodosum, and acute arthritis in the lower limbs, which 20-30 percent of patients with sarcoidosis develop. A positron emission tomography scan was performed and its results offered us diagnostic elements typical of a pulmonary inflammatory process far from the characteristics of a malignant neoplastic process(AU)

ABSTRACT Objective: To describe a case series of four (4) patients with hemolytic uremic syndrome due to Streptococcus pneumoniae in a level four complexity institution in the city of Bogotá, D.C., Colombia. Cases description: We describe cases of four patients who presented respiratory symptoms and fever. All four patients were in regular conditions on hospital admission, after which they required intensive care and ventilatory support. Upon admission, three cases showed evidence of pleuropulmonary complication. Penicillin-sensitive Streptococcus pneumoniae was isolated in all cases. All patients presented anemia, severe thrombocytopenia, schistocytes on peripheral blood smear, and hyperazotemia. They required blood transfusion and renal replacement therapy during their hospitalization. The patients were diagnosed with hemolytic uremic syndrome due to S. pneumoniae. Three of the four patients had a progressive recovery of the renal function and were discharged after an average of 36 days of hospital stay. The remaining patient had two amputations in the extremities due to thrombotic vascular complications and was discharged after 99 days of hospital stay, requiring hemodialysis every other day. Comments: Hemolytic uremic syndrome due to Streptococcus pneumoniae is a rare but severe complication of invasive pneumococcal disease. Complicated pneumonia is the main condition associated with this entity. It is noteworthy the short period in which these cases were presented, considering the low annual incidence of the disease.


RESUMO Objetivo: Descrever uma série de casos de quatro pacientes com síndrome hemolítico-urêmica por pneumococo em uma instituição de referência em Bogotá, Colômbia. Descrição dos casos: Descrevemos os casos de quatro pacientes que apresentaram sintomas respiratórios e febre. Todos estavam em estado geral regular à admissão hospitalar e necessitaram de cuidados intensivos e suporte ventilatório. Na admissão, em três dos casos foi evidenciada a complicação pleuropulmonar. Isolamento de Streptococcus pneumoniae sensível à penicilina foi realizado em todos os casos. Os quatro pacientes precisaram de transfusão sanguínea e terapia de reposição renal durante a hospitalização. Nos testes laboratoriais, observou-se anemia, trombocitopenia grave, presença de esquizócitos em esfregaço de sangue periférico e hiperazotemia. Com esse quadro, o diagnóstico foi de síndrome hemolítico-urêmica associada à infecção por S. pneumoniae. Houve recuperação progressiva da função renal em três dos quatro pacientes, que tiveram alta após 36 dias de internação hospitalar, em média. Um paciente teve complicações vasculares trombóticas, resultando em duas amputações nas extremidades, e teve alta após 99 dias de internação, com necessidade de hemodiálise em dias alternados. Comentários: A síndrome hemolítico-urêmica por Streptococcus pneumoniae é uma complicação rara, mas grave, da doença invasiva pneumocócica. A pneumonia complicada é a principal condição associada a essa entidade. Destaca-se o curto período em que esses casos foram apresentados, levando em conta a baixa incidência anual de síndrome hemolítico-urêmica.

ABSTRACT

Renal replacement therapy (RRT) is a crucial form of extracorporeal support in critical patients that develop acute kidney injury. This therapy allows to gain solute and water control when the kidneys are not capable of doing it. Nevertheless, RRT techniques are not without risks, complications and costs. Under this point of view, is fundamental to be conscious of the indications of timing and, most importantly, weaning of the RRT. The unnecesary extension of these techniques can lead to increase morbidity, hospital stay, health care costs and complications. Unfortunately, randomized controlled studies are scarce and this paucity of data has lead to different predictive models based on retrospective studies. Creatinine clearence, urinary output and other novel biomarkers has been used to identify the best moment to safely stop RRT. In this review, we summarize the available evidence about secure weaning of RRT and the potential of novel urinary and serum biomarkers that can be used to guide therapeutic decisions in the future. ABSTRACT Introduction Palliative care is an approach aimed at relieving suffering, controlling symptoms and seeking to improve quality of life. It must be offered in conjunction with standard treatment for any disease that threatens the continuation of life, such as a Covid-19 infection. Discussion The bioethical principles and strategies used by palliative medicine can assist nephrologists in the care of patients with renal dysfunction, who face the difficulties of isolation at the beginning and follow-up of dialysis in outpatient treatment, and those who are at risk for a more serious disease progress. Some of them: - a Shared decision making, which enables the patient and family to participate as facilitators in the systematization of the team's reasoning, in addition to respecting the principle of autonomy; - Symptom Management: which should be a priority to ensure relief of suffering even in times of social isolation; - Communication skills: making it possible to alleviate suffering in announcing bad news or complex decisions through communication techniques;; - Bereavement assistance: which in acute situations such as the pandemic, causing unexpected losses, the importance of sympathy from healthcare professionals becomes even greater. Conclusion The principles of palliative care are essential to face the challenges of a planet-wide crisis, which raises human suffering in all dimensions, and which requires the construction of strategies that can keep patients assisted, comfortable and with measures proportional to their clinical condition and preferences.


RESUMO Introdução O cuidado paliativo é uma abordagem voltada para alívio do sofrimento, controle de sintomas e melhora da qualidade de vida. Deve ser oferecido em conjunto com o tratamento padrão de qualquer doença que ameace a continuidade da vida, como, por exemplo, a infecção pela Covid-19. Discussão Os princípios bioéticos e as estratégias utilizadas pela medicina paliativa podem auxiliar os nefrologistas no cuidado dos pacientes com disfunção renal, que, além de serem do grupo de risco para evolução mais grave da infecção por coronavírus, enfrentam as dificuldades do isolamento no seguimento do tratamento dialítico e ambulatorial. Essas ferramentas são: I) tomada de decisão compartilhada, que proporciona a participação do paciente e dos familiares como facilitadores na sistematização do raciocínio da equipe, além de respeitar o princípio da autonomia; II) manejo de sintomas, que deve ser prioridade para a garantia do alívio do sofrimento mesmo em momento de isolamento social; III) habilidades em comunicação, sendo possível amenizar dificuldades em anunciar más notícias ou decisões complexas através de técnicas de comunicação; IV) assistência ao luto, em que, em situações agudas como a pandemia, de perdas inesperadas, a importância do acolhimento dos profissionais de saúde torna-se ainda maior. Conclusão Os princípios dos cuidados paliativos são essenciais para enfrentar os desafios de uma crise humanitária, que causa sofrimento ao ser humano em todas as dimensões e exige a construção de estratégias que possam manter os pacientes assistidos, confortáveis e com medidas proporcionais à sua condição clínica e às suas preferências.

ABSTRACT

Holmium laser lithotripsy (HLL) is one of the common surgical methods for urolithiasis. It causes minor surgical trauma, but complications are not rare. Extracorporeal membrane oxygenation (ECMO) treatment of sepsis is common, but venoarterial (VA)-ECMO treatment of urosepsis has not been reported yet. In this article, we reported a 67-year-old female patient with refractory septic shock caused by HLL under percutaneous nephroscope, involving breathing, heart, kidney and other organs, and organs support treatment was ineffective for the patient. Finally, we successfully treated the patient under VA-ECMO with continuous renal replacement therapy (CRRT). Combined ECMO and CRRT may provide a solution for addressing refractory sepsis. Here we present the case and review relevant literature, so as to provide a treatment strategy for patients with refractory urogenic sepsis and to reduce the mortality rate.

La Nefropatía mesoamericana, entidad de reciente definición que se caracteriza por episodios de Insuficiencia Renal Aguda y alta incidencia de Enfermedad Renal Crónica de etiología desconocida. Objetivo:Proporcionar información actualizada sobre Nefropatía mesoamericana de interés para la salud pública. Materiales y métodos: Búsqueda PubMed© utilizando términos Mesoamerican and Nephropathy‖ totalizando 67 resúmenes, seleccionando los que incluyen los últimos avances y puntos de interés clínico-epidemiológico.Resultados: Se postulan teorías basadas en datos de múltiples estudios transversales que tienen implicaciones de importancia para la salud pública y la mitigación en la incidencia de casos, así como para el manejo clínico; a pesar de eso no parece estar a la vista la identificación de la etiología. Conclusión: La falta de definición objetiva de caso sometido a riesgo de Insuficiencia Renal Crónica en zonas y ocupaciones de riesgo elevado para Nefropatía mesoamericana es uno de los obstáculos más importantes para desarrollar estrategias de prevención en salud pública...(AU)

ABSTRACT Objective: to analyze the factors related to the impaired comfort of chronic kidney diseases (CKD) patients on hemodialysis. Method: this is a cross-sectional study with 80 patients undergoing hemodialysis in a renal replacement therapy unit through interviews using two instruments, one for clinical and sociodemographic characteristics and the General Comfort Questionnaire, during the hemodialysis session. Mann-Whitney tests and the logistic regression model were used for data analysis. Results: the study found that being younger (p=0.045); being married (p=0.05); and absence of impaired physical mobility (p=0.007) were contributing factors for greater comfort in CKD patients on hemodialysis. Thus, when establishing the odds ratio, it was possible to observe that being 55 years of age or older, being single and having impaired physical mobility represents a 45.7% chance of developing this diagnosis. Conclusions: sociodemographic and clinical variables contribute to the study outcome, demanding attention during the planning of nursing interventions.


RESUMEN Objetivo: valorar los factores relacionados con la comodidad perjudicada del paciente renal crónico hemodialítico. Método: estudio transversal en el cual se entrevistaron 80 pacientes bajo tratamiento hemodialítico en una unidad de terapia renal sustitutiva, siendo aplicados dos instrumentos, las características sociodemográficas clínicas de salud y la Escala Comfort General, durante la sesión de hemodiálisis. Se utilizaron el test de Mann-Whitney y el modelo de regresión logística en el análisis de datos. Resultados: se observó que los factores que contribuyeron a una mayor sensación de comodidad en los pacientes renales crónicos hemodialíticos fueron: ser más joven (p=0,045); estar casado (p=0,05); y no tener la movilidad física perjudicada (p=0,007). En este sentido, al establecer la odds ratio, se observó que tener edad igual a 55 años o más, estar soltero y presentar movilidad física perjudicada representa un 45,7% de probabilidad de desarrollar este diagnóstico. Conclusiones: las variables sociodemográficas y clínicas contribuyen al desenlace en estudio, requiriendo atención ante la planificación de las intervenciones de enfermería.

Método: estudo transversal com 80 pacientes sob tratamento hemodialítico em unidade de terapia renal substitutiva por meio de entrevista com aplicação de dois instrumentos, características sociodemográficas clínicas de saúde e Escala de Conforto Geral, durante sessão de hemodiálise. Utilizou-se os testes de Mann-Whitney e modelo de regressão logística para análise dos dados. Resultados: observou-se que ser mais jovem (p=0,045); estar casado (p=0,05); e ausência de mobilidade física prejudicada (p=0,007) foram fatores contribuintes para maior conforto nos pacientes renais crônicos hemodialíticos. Nesse sentido, ao estabelecer a odds ratio, foi possível observar que ter idade maior ou igual a 55 anos, estar solteiro e apresentar mobilidade física prejudicada representa 45,7% de chance de desenvolver esse diagnóstico. Resumo A doença renal crônica (DRC) acarreta ao indivíduo uma sobrecarga de cuidados indispensáveis ao alcance das metas terapêuticas e qualidade de vida. O controle nutricional, o regime medicamentoso e as terapias de substituição renal são exemplos que requerem participação ativa. Assim, foi realizada revisão integrativa de artigos científicos para identificar os que versam sobre envolvimento e participação do paciente em seus cuidados na DRC. Os descritores "Chronic kidney disease"; "Self-Care" and "Patient Participation" foram aplicados nas bases CINHAL, Bireme e Medline. Selecionamos 21 publicações entre 2012 e 2016. A entrevista semiestruturada destacou-se como método de coleta de dados, com a aplicação da análise temática fenomenológica. Conforme a predominância do assunto tratado, os manuscritos foram alocados em quatro eixos temáticos: Manejo da doença renal e seu tratamento; Envolvimento na tomada de decisão; Plano avançado de cuidados; e, Diálise peritoneal domiciliar. Verificamos que abordagens envolvendo pessoas transplantadas ou nos primeiros estágios da doença são pouco explorados com a aplicação do método qualitativo e, ainda, consideramos que pessoas com DRC devem ser instigadas no envolvimento ativo de seus próprios cuidados, necessitando de conhecimento, motivação e suporte dos profissionais de saúde.


Abstract The treatment of chronic kidney disease (CKD) places a major burden on patients and their families. Interventions such as nutritional management, medication regimen, and renal replacement therapies require active patient participation. An integrative literature review was carried out to identify articles on the engagement and participation of people with CKD in their care. The Medical Subject Headings (MeSH) "Kidney Failure, Chronic", "Self Care", and "Patient Participation" were used to conduct a search on the following databases: Cumulative Index to Nursing and Allied Health Literature (CINAHL), the U.S. National Library of Medicine® (Medline/PubMed), Biblioteca Virtual em Saúde (Bireme). A total of 21 articles published between 2012 and 2016 were selected. The most commonly used data collection and analysis techniques were semi-structured interviews and phenomenological thematic analysis, respectively. The articles were categorized into the following thematic groups: illness management and treatment; involvement in the decision-making process; advanced care plan; and home peritoneal dialysis. We found that there is a lack of qualitative research in certain areas, namely kidney transplant recipients and people with initial stages of CKD. People with CKD should be encouraged to actively engage in their own care, which in turn requires the knowledge, motivation and support of health professionals.

ABSTRACT

Background: In critical patients with acute renal failure, intermittent diffusive renal replacement techniques cause hemodynamic problems due to their high depurative efficiency. This situation is avoided using continuous low efficiency therapies, which are expensive, prevent patient mobilization and add hemorrhagic risk due to systemic anticoagulation. Intermittent and prolonged hemodiafiltration (HDF) has the depurative benefits of diffusion, plus the positive attributes of convection in a less expensive therapy. Aim: To report our experience with intermittent and prolonged on-line HDF in critically ill patients. Patients and Methods: During 2016, HDF therapies performed on critical patients with indication of renal replacement therapy were characterized. The hemodynamic profile was evaluated (doses of noradrenaline, blood pressure, heart rate and perfusion parameters). Results: Fifty-one therapies were performed in 25 critical patients, aged 58 ± 11 years (28% women), with an APACHE II score of 22.1 ±10. The average time of the therapies was 4.15 hours (range 3-8 hours), the replacement volume was 75 ± 18 mL/kg/h and ultrafiltration rate was 226 ± 207 mL/h. The mean initial, maximum and final noradrenaline doses were 0.07 ± 0.1, 0.13 ±0.18 and 0.09 ±0.16 μg/kg/min respectively. No differences between patients with low, medium and high doses of noradrenaline or dose increases during therapy, were observed. The greatest decrease in mean arterial pressure was 15.3% and the maximum increase in heart rate was 12.8%. Anticoagulation was not required in 88% of therapies. Conclusions: High-volume intermittent or prolonged HDF is an effective therapy in critical patients, with good hemodynamic tolerability, lower costs and avoidance of systemic anticoagulation risks.

RESUMO Objetivo: Avaliar a associação entre a presença de lesão renal aguda, por meio do escore pediatric Risk, Injury, Failure, Loss and End Stage Renal Disease, e mortalidade em unidade de terapia intensiva pediátrica. Métodos: Estudo de coorte retrospectivo que incluiu todas as crianças internadas em uma unidade de terapia intensiva pediátrica de um hospital de referência no Brasil, entre os meses de janeiro e dezembro de 2016. Os pacientes foram triados quanto à presença de lesão renal aguda por meio do escore pediatric Risk, Injury, Failure, Loss and End Stage Renal Disease. Elas foram subdivididas segundo os estádios risco, lesão e falência renal. Resultados: A amostra foi composta por 192 crianças, das quais 45,8% desenvolveram lesão renal aguda, tendo a identificação desta lesão ocorrido em até 72 horas após a admissão em 79,5% dos casos. Pacientes com lesão renal aguda apresentaram 3,74 mais chances de morrer (p = 0,01) em comparação às crianças do grupo controle. Pacientes com falência renal apresentaram mortalidade 8,56 vezes maior que a do restante da amostra (p < 0,001). As variáveis que apresentaram associação com os estádios de lesão renal aguda foram: uso de fármacos nefrotóxicos (p = 0,025), terapia de substituição renal (p < 0,001), uso de fármacos vasoativos (p < 0,001), escore Pediatric Risk of Mortality 2 (p = 0,023), sobrecarga de fluidos (p = 0,005), tempo de internação na unidade de terapia intensiva pediátrica (p = 0,001) e morte (p < 0,001). Conclusão: Neste estudo, o escore pediatric Risk, Injury, Failure, Loss and End Stage Renal Disease mostrou-se ferramenta útil para a identificação precoce de crianças com lesão renal aguda grave, mostrando associação com a mortalidade. Sugerimos seu uso rotineiro na admissão de pacientes à unidade de terapia intensiva pediátrica.


ABSTRACT Objective: To evaluate the association between acute kidney injury through the pediatric Risk, Injury, Failure, Loss and End Stage Renal Disease score and mortality in a pediatric intensive care unit. Methods: This retrospective cohort study assessed all children admitted to the pediatric intensive care unit of a reference hospital in Brazil from January to December 2016. Patients were screened for the presence of acute kidney injury through the pediatric Risk, Injury, Failure, Loss and End Stage Renal Disease score. Patients were subdivided into the stages of Risk, Injury and Kidney Failure. Results: The sample comprised 192 children, of whom 45.8% developed acute kidney injury, with 79.5% of the cases identified up to 72 hours after admission. Patients with acute kidney injury showed a 3.74 increase risk of death (p = 0.01) than the control group. Patients with kidney failure had a mortality rate that was 8.56 times greater than that of the remaining sample (p < 0.001). The variables that were associated with the stages of acute kidney injury were nephrotoxic drugs (p = 0.025), renal replacement therapy (p < 0.001), vasoactive drugs (p < 0.001), pediatric risk of mortality 2 score (p = 0.023), fluid overload (p = 0.005), pediatric intensive care unit length of stay (p = 0.001) and death (p < 0.001). Conclusion: In this study, the pediatric Risk, Injury, Failure, Loss and End Stage Renal Disease score proved to be a useful tool for the early identification of severely ill children with acute kidney injury, showing an association with mortality. We thus suggest its use for pediatric intensive care unit patient admission.

RESUMO Objetivo: Avaliar se, em comparação ao início tardio, o início precoce da terapia de substituição renal se associa com menor mortalidade em pacientes com lesão renal aguda. Métodos: Conduzimos uma revisão sistemática e metanálise de ensaios clínicos randomizados e controlados, que compararam terapia de substituição renal com início precoce àquela com início tardio em pacientes com lesão renal aguda, sem sintomas relacionados à insuficiência renal aguda que oferecessem risco à vida, como sobrecarga hídrica ou distúrbios metabólicos. Dois investigadores extraíram os dados a partir de estudos selecionados. Utilizaram-se a ferramenta Cochrane Risk of Bias, para avaliar a qualidade dos estudos, e a abordagem Grading of Recommendations Assessment, Development and Evaluation (GRADE), para testar a qualidade geral da evidência. Resultados: Incluíram-se seis estudos clínicos randomizados e controlados (1.292 pacientes). Não houve diferença estatisticamente significante entre o início precoce e tardio da terapia de substituição renal, no que se referiu ao desfecho primário (OR 0,82; IC95% 0,48 - 1,42; p = 0,488). Foi maior o risco de infecção da corrente sanguínea relacionada ao cateter quando a terapia de substituição renal foi iniciada precocemente (OR 1,77; IC95% 1,01 - 3,11; p = 0,047). A qualidade da evidência gerada por nossa metanálise para o desfecho primário foi considerada baixa, em razão do risco de viés dos estudos incluídos e da heterogeneidade entre eles. Conclusão: O início precoce da terapia de substituição renal não se associou com melhora da sobrevivência. Entretanto, a qualidade da evidência atual é baixa, e os critérios utilizados para início precoce e tardio da terapia de substituição renal foram demasiadamente heterogêneos entre os estudos.


ABSTRACT Objective: To evaluate whether early initiation of renal replacement therapy is associated with lower mortality in patients with acute kidney injury compared to delayed initiation. Methods: We performed a systematic review and meta-analysis of randomized controlled trials comparing early versus delayed initiation of renal replacement therapy in patients with acute kidney injury without the life-threatening acute kidney injury-related symptoms of fluid overload or metabolic disorders. Two investigators extracted the data from the selected studies. The Cochrane Risk of Bias Tool was used to assess the quality of the studies, and the Grading of Recommendations Assessment, Development and Evaluation (GRADE) approach was used to test the overall quality of the evidence. Results: Six randomized controlled trials (1,292 patients) were included. There was no statistically significant difference between early and delayed initiation of renal replacement therapy regarding the primary outcome (OR 0.82; 95%CI, 0.48 - 1.42; p = 0.488), but there was an increased risk of catheter-related bloodstream infection when renal replacement therapy was initiated early (OR 1.77; 95%CI, 1.01 - 3.11; p = 0.047). The quality of evidence generated by our meta-analysis for the primary outcome was considered low due to the risk of bias of the included studies and the heterogeneity among them. Conclusion: Early initiation of renal replacement therapy is not associated with improved survival. However, the quality of the current evidence is low, and the criteria used for -early- and -delayed- initiation of renal replacement therapy are too heterogeneous among studies.

RESUMO Objetivo: Avaliar a capacidade instalada de terapêutica substitutiva da função renal e práticas locais na abordagem da lesão renal aguda em centros participantes do estudo BaSICS. Métodos: Um questionário foi enviado aos coordenadores de 61 unidades de terapia intensiva participantes de um ensaio clínico randomizado brasileiro. Um total de 124 médicos respondeu ao questionário. Resultados: No momento do questionário, 15% dos pacientes nas unidades de terapia intensiva participantes encontravam-se em terapêutica substitutiva da função renal. Todas as unidades de terapia intensiva dispunham de, pelo menos, um método de terapêutica substitutiva da função renal. Métodos contínuos estavam mais disponíveis em unidades privadas do que nas públicas. O tempo entre indicação do método e início da terapia foi maior em unidades de terapia intensiva públicas do que nas privadas. Os principais obstáculos para início do método em unidades de terapia intensiva públicas incluíam disponibilidade de maquinário e pessoal, enquanto que o principal gargalo em unidades de terapia intensiva privadas foi a avaliação do nefrologista. Parte importante dos médicos avaliados mudaria sua prática de manuseio de terapêutica substitutiva da função renal caso não houvesse limitação de métodos de terapêutica substitutiva da função renal em suas unidades. Conclusão: Existe ampla variedade na disponibilidade de recursos para terapêutica substitutiva da função renal e nas práticas de manuseio da lesão renal aguda em unidades de terapia intensiva brasileiras. Estas informações devem ser levadas em conta ao planejarem-se ensaios clínicos sobre o assunto no contexto brasileiro.


ABSTRACT Objective: To investigate the existing capacity for renal replacement therapy and site-specific practices for managing acute kidney injury at centers participating in the BaSICS trial. Methods: A questionnaire was provided to the chairs of 61 intensive care units enrolled in a randomized clinical trial in Brazil. A total of 124 physicians completed the questionnaire. Results: Approximately 15% of the patients admitted to the analyzed intensive care units received renal replacement therapy at the time of data collection. At least one renal replacement method was available in all of the analyzed units. Continuous methods were available more frequently at the private units than at the public units. The time from indication to onset of treatment was longer at the public units than at private units. The main obstacles to treatment initiation at public intensive care units were related to the availability of equipment and personnel, while the main bottleneck at private units was the nephrologist assessment. A considerable proportion of the participants stated that they would change their approach to renal replacement therapy if there were no limitations on the availability of methods in their units. Conclusion: There was wide variation in the availability of resources for renal replacement therapy and in the management of acute kidney injury in Brazilian intensive care units. This information should be taken into account when planning clinical trials focused on this topic in Brazil.

The complexity, implicit in the implementation of both techniques, can be overwhelming. This review aims to explain the most important aspects of continuous renal replacement therapy in a patient with extracorporeal support.

RESUMO Objetivo: Investigar os fatores clínicos e laboratoriais associados com a variação dos níveis séricos de sódio durante terapia renal substitutiva contínua e avaliar se a fórmula de mixagem perfeita pode prever a variação do sódio nas 24 horas. Métodos: A partir de uma base de dados coletada de forma prospectiva, recuperamos e analisamos os dados referentes a 36 sessões de terapia renal substitutiva realizadas em 33 pacientes, nas quais a prescrição de afluentes permaneceu inalterada durante as primeiras 24 horas. Aplicamos um modelo linear misto para investigar os fatores associados com grandes variações dos níveis séricos de sódio (≥ 8mEq/L) e geramos um gráfico de Bland-Altman para avaliar a concordância entre as variações previstas e observadas. Resultados: Nas sessões de terapia renal substitutiva de 24 horas identificamos que SAPS 3 (p = 0,022) e hipernatremia basal (p = 0,023) foram preditores estatisticamente significantes de variações séricas do sódio ≥ 8mEq/L na análise univariada, porém apenas hipernatremia demonstrou uma associação independente (β = 0,429; p < 0,001). A fórmula de mixagem perfeita para previsão do nível de sódio após 24 horas demonstrou baixa concordância com os valores observados. Conclusões: A presença de hipernatremia por ocasião do início da terapia renal substitutiva é um fator importante associado com variações clinicamente significativas dos níveis séricos de sódio. O uso de citrato 4% ou da fórmula A de ácido citrato dextrose 2,2% como anticoagulantes não se associou com variações mais acentuadas dos níveis séricos de sódio. Não foi viável desenvolver uma predição matemática da concentração do sódio após 24 horas.


ABSTRACT Objective: The aim of this study was to investigate the clinical and laboratorial factors associated with serum sodium variation during continuous renal replacement therapy and to assess whether the perfect admixture formula could predict 24-hour sodium variation. Methods: Thirty-six continuous renal replacement therapy sessions of 33 patients, in which the affluent prescription was unchanged during the first 24 hours, were retrieved from a prospective collected database and then analyzed. A mixed linear model was performed to investigate the factors associated with large serum sodium variations (≥ 8mEq/L), and a Bland-Altman plot was generated to assess the agreement between the predicted and observed variations. Results: In continuous renal replacement therapy 24-hour sessions, SAPS 3 (p = 0.022) and baseline hypernatremia (p = 0.023) were statistically significant predictors of serum sodium variations ≥ 8mEq/L in univariate analysis, but only hypernatremia demonstrated an independent association (β = 0.429, p < 0.001). The perfect admixture formula for sodium prediction at 24 hours demonstrated poor agreement with the observed values. Conclusions: Hypernatremia at the time of continuous renal replacement therapy initiation is an important factor associated with clinically significant serum sodium variation. The use of 4% citrate or acid citrate dextrose - formula A 2.2% as anticoagulants was not associated with higher serum sodium variations. A mathematical prediction for the serum sodium concentration after 24 hours was not feasible.

RESUMO Objetivo: Identificar fatores prognósticos relacionados com a mortalidade ou com a não recuperação da função renal. Métodos: Estudo monocêntrico, prospectivo, realizado em um serviço de medicina intensiva de um hospital universitário, entre 2012 e 2015. Incluíram-se doentes com lesão renal aguda em suporte renal contínuo. Foram coletados parâmetros clínicos e analíticos, assim como foi investigado o motivo para o início e o término do suporte renal. Resultados: Foram incluídos 41 doentes, 43,9% deles com sepse. O Simplified Acute Physiology Score II (SAPS-II) foi de 56, com mortalidade prevista de 59,8% e verificada de 53,7%. A etiologia da lesão renal aguda foi principalmente multifatorial (56,1%). Os sobreviventes apresentaram menor balanço hídrico acumulado (mediana de 3.600mL com intervalo interquartil de 1.175 - 8.025 versus 12.000mL [6.625 - 17.875] e p = 0,004. Os doentes que recuperaram função renal apresentaram SAPS II mais baixo do que os que não recuperaram (51,0 [45,8 - 56,2] versus 73 [54 - 85]; p = 0,005), assim como menor balanço hídrico (3850 [1.425 - 8.025] versus 11.500 [6.625 - 16.275]; p = 0,004). Conclusão: SAPS II na admissão e balanço hídrico acumulado durante o suporte renal foram fatores de risco para mortalidade e para a não recuperação da função renal em doentes graves com lesão renal aguda e necessidade de suporte renal.


ABSTRACT Objective: Identify prognostic factors related to mortality and non-recovery of renal function. Methods: A prospective single-center study was conducted at the intensive care medicine department of a university hospital between 2012 and 2015. Patients with acute kidney injury receiving continuous renal replacement therapy were included in the study. Clinical and analytical parameters were collected, and the reasons for initiation and discontinuation of renal replacement therapy were examined. Results: A total of 41 patients were included in the study, of whom 43.9% had sepsis. The median Simplified Acute Physiology Score II (SAPSII) was 56 and the mortality was 53.7%, with a predicted mortality of 59.8%. The etiology of acute kidney injury was often multifactorial (56.1%). Survivors had lower cumulative fluid balance (median = 3,600mL, interquartile range [IQR] = 1,175 - 8,025) than non-survivors (median = 12,000mL, IQR = 6,625 - 17,875; p = 0.004). Patients who recovered renal function (median = 51.0, IQR = 45.8 - 56.2) had lower SAPS II than those who do not recover renal function (median = 73, IQR = 54 - 85; p = 0.005) as well as lower fluid balance (median = 3,850, IQR = 1,425 - 8,025 versus median = 11,500, IQR = 6,625 - 16,275; p = 0.004). Conclusions: SAPS II at admission and cumulative fluid balance during renal support therapy were risk factors for mortality and non-recovery of renal function among critically ill patients with acute kidney injury needing renal replacement therapy.

ABSTRACT

A pesar de los avances en el desarrollo de las terapias de reemplazo renal, la mortalidad de la falla renal aguda permanece elevada, especialmente, cuando se manifiesta simultáneamente con fallas orgánicas distantes, como es en el caso del síndrome de distrés respiratorio agudo. Se revisa la relación bidireccional deletérea entre pulmón y riñón, en el escenario de disfunción orgánica, la cual presenta aspectos clínicos relevantes de conocer. Se discuten los efectos renales del síndrome de distrés respiratorio agudo y del uso de la ventilación mecánica a presión positiva, siendo el daño inducido por este (ventilator induced lung injury) uno de los modelos utilizado frecuentemente para el estudio de la interacción pulmón-riñón. Se enfatiza el rol de la falla renal inducida por la ventilación mecánica (ventilator-induced kidney injury) en la patogenia de la falla renal aguda. Asimismo se analizan las repercusiones pulmonares de la falla renal aguda, reconociéndose que esta condición patológica induce un incremento en la permeabilidad vascular pulmonar, inflamación y alteración de los canales de sodio y agua del epitelio alveolar, entre otros efectos. Este modelo conceptual puede ser la base para el desarrollo de nuevas estrategias terapéuticas a utilizar en el paciente con síndrome de disfunción orgánica múltiple.


Despite advances in the development of renal replacement therapy, mortality of acute renal failure remains high, especially when occurring simultaneously with distant organic failure as it is in the case of the acute respiratory distress syndrome. In this update, birideccional deleterious relationship between lung and kidney on the setting of organ dysfunction is reviewed, which presents important clinical aspects of knowing. Specifically, the renal effects of acute respiratory distress syndrome and the use of positive-pressure mechanical ventilation are discussed, being ventilator induced lung injury one of the most common models for studying the lung-kidney crosstalk. The role of renal failure induced by mechanical ventilation (ventilator-induced kidney injury) in the pathogenesis of acute renal failure is emphasized. We also analyze the impact of the acute renal failure in the lung, recognizing an increase in pulmonary vascular permeability, inflammation, and alteration of sodium and water channels in the alveolar epithelial. This conceptual model can be the basis for the development of new therapeutic strategies to use in patients with multiple organ dysfunction syndrome.
